Model 20KNKS-02-000278 is a Signo 20K Keypad Mullion Pigtail Reader made by HID Global. The keypad lets a door ask for a PIN as well as a card, a simple way to add a second factor at entrances that need it. The narrow mullion design suits door frames where a wider reader will not fit. A pigtail lead makes it quick to splice into the reader cable.
